490.121 FORMS. 1. The secretary of state may prescribe and furnish on request forms including but not limited to the following: a. A foreign corporation's application for a certificate of authority to transact business in this state. b. A foreign corporation's application for a certificate of withdrawal. c. The biennial report. If the secretary of state so requires, use of these listed forms prescribed by the secretary of state is mandatory. 2.
