600.6 - ATTACHMENTS TO AN ADOPTION PETITION.

600.6 ATTACHMENTS TO AN ADOPTION PETITION. An adoption petition shall have attached to it the following: 1. A certified copy of the birth certificate showing parentage of the person to be adopted or, if such certificate is not available, a verified birth record. 2. A copy of any order terminating parental rights with respect to the person to be adopted. 3. Any written consent and verified statement required under section 600.7, except the consent required under subsection 1, paragraph "d", of that section. 4. Any preplacement investigation report that has been prepared at the time of filing pursuant to section 600.8. 5.
