Chapter 40.--INSURANCE Article 11.--GENERAL PROVISIONS RELATIVE TO CASUALTY, SURETY AND FIDELITY COMPANIES

      40-1127.   Same; content of reports.The reports required by subsection (a) of K.S.A.40-1126 shall contain: (a) The insured's name andlicense number issued by the state board of healing arts; (b) date ofoccurrence which created the action; and (c) the name of the plaintiff and theinjured party, if different.
