40-2130. Same; form directing withholding or withdrawal of life-sustainingprocedures to be provided applicants for coverage; retention upon execution.The association or a member insurer thereof shall provideevery applicant for health coverage under the provisions of this act with aform for making a declaration directing the withholding or withdrawal oflife-sustaining procedures in a terminal condition in substantial conformancewith subsection (c) of K.S.A. 65-28,103, and amendments thereto. If suchapplicant elects to execute such declaration the applicant shall submit a copyof such declaration to the association or memberinsurer thereof, and such copy shall be retained and made a part of theapplicant's permanent records.
