40-2507. Same; service contracts.(a) Upon acceptance by an automobile club of an application formembership, the automobile club shall promptly issue a service contract,which shall consist of a written description of the services and featuresto be provided and a membership card, and which shall contain theinformation specified in subsection (c) hereof.
(b) The service contract shall not be issued or delivered in this stateuntil the form thereof and the charges made, as well as any revisionsthereof, are filed with the commissioner.
(c) A service contract shall not be issued or delivered in this stateunless it contains the following: (1) The corporate or other name of theclub.
(2) The location of its home office and its usual place of business inthis state, if any, giving street number and city.
(3) Provisions clearly and plainly specifying: (A) A description of theservices or benefits to which the member is entitled.
(B) The territory wherein such services are to be rendered.
(C) The date when such service will commence and terminate.
