      40-37a05.   Disclosure of producer-insurerrelationship to insured.The producer, prior to the effective date of the policy,shalldeliver written notice to the prospective insured disclosing the relationshipbetween the producer and the controlled insurer; exceptthat, if the business isplaced through a subproducer who is not a controllingproducer,the controlling producershall retain in such controlling producer's records a signed commitmentfrom the subproducer that suchsubproducer isaware of the relationship between the insurer and theproducer and that thesubproducer has or will notify the insured.
