49-612. Same; periodic reports, contents; report after completion ofmining operation.(a) An operator shall file with the director a periodic report foreach site under registration. The report shall make reference to the mostrecent registration of the mine site and shall show:
(1) The location and extent of all surface land area on the mine siteaffected by mining during the period covered by the report.
(2) The extent to which removal of mineral products from all or any part ofthe affected land has been completed.
(b) A report shall also be filed within 90 days after completion of allsurface mining operations at the site regardless of the date of the lastpreceding report. Forms for the filing of periodic reports required by thissection shall be provided by the director.
