      65-1,244.   Removal of information from system;form.A child's parent or legal guardian who wants informationconcerning the child removed from the birth defects information system shallrequest from the local health department or the child's physician a formprepared by the secretary. On request, a local health department or physicianshall provide the form to the child's parent or legal guardian. The individualproviding the form shall discuss with the child's parent or legal guardian theinformation contained in the system. If the child's parent or legal guardiansigns the form, the local health department or physician shall forward it tothe secretary. On receipt of the signed form, the secretary shall remove fromthe system any information that identifies the child.
