      65-34,104.   Same; notification to department of tank's existence; form.(a) Each owner of a storage tank shall notify the departmentof the tank's existence, including age, size, type, location, associatedequipment and uses.

      (b)   In addition and to the extent known, each owner of an undergroundstorage tank which has not been removed, but was taken out of service afterJanuary 1, 1974, and prior to May 8, 1986, shall notify the department ofthe date the tank was taken out of operation, the age of the tank on thedate taken out of operation, the capacity, type and location of the tank,and the type and quantity of substances stored in the tank on the datetaken out of operation.

      (c)   Notice shall be made on an approved form provided by the department.
