Chapter 65.--PUBLIC HEALTH Article 4.--HOSPITALS AND OTHER FACILITIES

      65-428.   Application for license;contents.An application for a license shall be made to the licensing agency uponforms provided by it and shall contain such information as the licensingagency may reasonably require, which may include affirmative evidence ofability to comply with such reasonable standards and rules and regulations asare lawfully adopted hereunder.
