Page 1 of 1 17.558 Issuance of approvals. (1) The board may issue, refuse to issue, reissue, or renew a provider approval, or may probate, suspend, or revoke the approval of a provider. (2) The board shall revoke the approval of a provider while his or her approval is suspended. Effective: April 11, 2000
