Report forms--how provided, contents--approval by superintendent.

43.251. 1. The Missouri division of highway safety shall prepare andupon request supply to police departments, sheriffs, and other appropriateagencies or individuals forms for written accident reports as required bysection 43.250 and this section. Reports shall call for sufficiently detailedinformation to disclose, with reference to a vehicle accident, the cause,conditions then existing and the persons and vehicles involved.

2. Every written or computer-generated accident report required to bemade shall be submitted on the appropriate form or in the appropriate computerformat approved by the superintendent of the Missouri state highway patrol andshall contain all the information required therein unless not available.
