Form of ballot.

206.070. The question shall be submitted in substantially the followingform:

Shall there be organized in the counties of ......, state of Missouri, ahospital district for the establishment and operation of a hospital to belocated at* ....... in ....... County, Missouri, and having the power toimpose a property tax not to exceed the annual rate of fifty cents on thehundred dollars assessed valuation without voter approval, and such additionaltax as may be approved hereafter by vote thereon to be known as ".......Hospital District", as prayed for by petition filed with the county clerk of....... County, Missouri, on the .... day of ......, 20......?
