Content of withholding forms.

285.304. The content of the withholding form shall be determined bythe director of the department of revenue, in consultation with thedepartment of social services, but, at a minimum, the form shall includethe name, address and Social Security number of the employee, and the nameand address of, and identifying number assigned to the employer undersection 6109 of the Internal Revenue Code of 1986, as amended. If theemployer chooses to submit a form other than the federal W-4 withholdingform, the form shall also include the date the employee signed the W-4 formor the date the employer hired the employee as defined in section 285.300.
