Application form, contents--fee.

332.141. Applications for dental examination andregistration shall be in writing and on forms furnished to theapplicant and shall contain the applicant's statements showingthe applicant's education and such other pertinent information asthe board may require. Each application shall contain astatement that it is made under oath or affirmation and that itsrepresentations are true and correct to the best knowledge andbelief of the person signing same, subject to the penalties ofmaking a false affidavit or declaration, and shall be accompaniedby a dentist examination fee.
