Affidavit of retirement.

334.600. Any person licensed to practice as a physical therapist inthis state who retires from the practice shall file with the board anaffidavit, on a form furnished by the board, which states the date on whichthe person retired from the practice and such other facts as tend to verifythe retirement as the board deems necessary, and if the person thereafterreengages in the practice, the person shall register as provided bysections 334.500 to 334.620.
