Designation nontransferable, site specific--annual affidavitrequired--notice of certain changes, required when.

354.658. 1. The designation of essential community provider shall notbe transferable to another physician, health care provider or entity.

2. The designation of essential community provider shall be physicianand site specific and shall not be effective at a site that has not been sodesignated pursuant to subdivision (8) of section 354.650.

3. The essential community provider shall submit an annual affidavit tothe department stating that such community provider continues to meet thequalifications for which the provider was certified. The essential communityprovider shall notify the department of health and senior services withinthirty days of any changes which may affect the certification requirements ofthe essential community provider.
