374.505. 1. An applicant for a certificate shall:
(1) Submit an application to the department of insurance, financialinstitutions and professional registration; and
(2) Pay to the department of insurance, financial institutions andprofessional registration the application fee established by the departmentthrough regulation.
2. The application shall:
(1) Be on a form and accompanied by any reasonably related supportingdocumentation that the department of insurance, financial institutions andprofessional registration requires; and
(2) Be signed and verified by the applicant.
3. The application fee required under this subsection shall besufficient to pay for the administrative cost of the certification program andany other cost associated with carrying out the provisions of sections 374.500to 374.515.
