Plan to file copy of bylaws, coverage and agreements with director.

376.1007. At the time application for a certificate ofauthority is made, the multiple employer self-insured health planshall file with the director a copy of the plan's bylaws, allschedules of benefits, and all management, administration, andtrust agreements which the plan has made or proposes to make forthe conduct of its business and affairs. Any proposed changes oramendments to the foregoing shall also be filed with the directorat least sixty days prior to the proposed effective date.
