Definitions for group health conversion policy requirements.

376.395. As used in sections 376.395 to 376.404 thefollowing terms shall mean:

(1) "Group policy", a group health insurance policy issuedby an insurance company and a group contract issued by a healthservice corporation, a health maintenance organization or asimilar corporation or organization;

(2) "Group policyholder", the entity purchasing the grouppolicy from the insurer;

(3) "Individual policy" or "converted policy", anindividual health insurance policy issued by an insurancecompany or an individual health services contract issued by ahealth service corporation, a health maintenance organizationor a similar corporation or organization;

(4) "Insurer", the entity issuing a group policy or anindividual or converted policy;

(5) "Medicare", Title XVIII of the United States SocialSecurity Act as added by the Social Security Amendments of 1965or as later amended or superseded;

(6) "Premium", any premium or other consideration payablefor coverage under a group or individual policy.
