376.678. 1. Notwithstanding any other provision of law, alllife insurance companies licensed to do business in this stateshall furnish at least annually to each individual whole lifepolicyholder and individual deferred annuity contractholder, astatement or notice providing sufficient information to permitidentification of the policy or contract. Such identificationnotice shall include the name of the policyholder orcontractholder, the policy or contract number and any premiumpayable.

2. Upon the request of the policyholder or an authorizedrepresentative, such company shall, within a reasonable period oftime, furnish applicable information relating to the policy orcontract, such as, type of plan, amount of insurance, policyvalues, policy loans or dividends.
