Report, when required, contents.

444.630. 1. The operator shall file a report with thecommission within sixty days after the date of expiration of apermit stating the exact number of acres of land affected by theoperation, the extent of the reclamation already accomplished,and such other information as may be required by the commission.

2. The operator shall file another report within thirty daysafter grading is completed or sixty days after twelve months fromthe date of expiration of a permit stating the extent of gradingand such other information as may be required by the commission.

3. The operator shall file a planting report with thecommission within thirty days after revegetation is completed oris to be completed, showing the extent of revegetation and suchother information as may be required by the commission.
