8-1005. Application; contents.Each application for such license shall be made in writing, under oath, to the director in such form as he or she may prescribe. The application shall state the full name and business address of:(1) The proprietor, if the applicant is an individual;(2) Every member, if the applicant is a partnership, limited liability company, or association, except that if the applicant is a joint-stock association having fifty or more members, the name and business address need be given only of the association and each officer and director thereof;(3) Every trustee and officer if the applicant is a trust; or(4) The corporation and each officer and director thereof, if the applicant is a corporation.
