641A.270  Automatic revocation of license for failure to pay biennial fee for registration; reinstatement.  Failure to pay the fee for renewal automatically effects a revocation of the license on the date of expiration of the license. The license may not be reinstated except upon:

      1.  Written application;

      2.  Submission of evidence of the completion of the required continuing education for the period the license was revoked; and

      3.  The payment of the fee for renewal and the fee for reinstatement required by this chapter.
