644.295  Duplicate license; fee.

      1.  A person licensed pursuant to this chapter shall obtain a duplicate of that license if the:

      (a) Original was destroyed, misplaced or mutilated; or

      (b) Name or address of the licensee has changed.

      2.  To obtain a duplicate license a person must:

      (a) File an affidavit with the Board, on the form prescribed by the Board, which states that the original license was destroyed, misplaced or mutilated or that the person’s name or address has changed; and

      (b) Pay a fee of $25.
