689C.160  Carrier must uniformly apply requirements to determine whether to provide coverage.  The requirements used by a carrier serving small employers to determine whether to provide coverage to a small employer, including, without limitation, standards for medical underwriting, requirements for minimum participation of eligible employees and minimum employer’s contributions, must be applied uniformly among all small employers with the same number of eligible employees applying for coverage or receiving coverage from the carrier.
