Section 328-F:10 Records of Licensees and Disciplinary History. - New Hampshire Statutes

   I. Each governing board shall maintain a list of living or deceased persons who have ever been licensed to practice. The list shall show the licensee's last known place of employment, last known place of residence, the number of the license, and the most recent date of issuance, renewal, or reinstatement. No information on the list shall be transferred to another entity for commercial use without the permission of the person to whom the information applies.
   II. Each governing board shall maintain a list of persons against whom the board has taken any disciplinary action. This list shall include the name of the person, the reason for the disciplinary action, the date of the disciplinary action, and the nature of the disciplinary action.
