4171 - Records; duties of physicians and others to furnish information.
§ 4171. Records; duties of physicians and others to furnish information. 1. Physicians, nurse-midwives, funeral directors, undertakers and informants, and all other persons having knowledge of the facts, are hereby required to supply, upon a form provided by the commissioner or upon the original certificate, such information as they may possess regarding any birth or death upon demand of the commissioner, in person, by mail, or through the registrar. 2. No forms shall be used other than those supplied by the commissioner. 3. All certificates, either of birth or death, shall be written legibly, in durable black ink, and no certificate shall be held to be complete and correct that does not supply all of the items of information called for therein, or satisfactorily account for their omission.
