365-D - Early and periodic screening diagnosis and treatment outreach demonstration projects.

* § 365-d. Early and periodic screening diagnosis and treatment outreach demonstration projects. 1. In accordance with applicable federal requirements and to the extent that federal matching funds are available, the department is authorized, subject to the approval of the director of the budget, to enter into contractural agreements with public and/or private organizations to develop and implement a maximum of five early and periodic screening diagnosis and treatment outreach demonstration projects. Such outreach demonstration projects may serve a social services district, more than one district, or part of a district. In approving such outreach demonstration project plans the department shall consider the effectiveness of each proposed outreach demonstration project in meeting the following objectives: (a) increasing the participation of eligible households in the early and periodic screening diagnosis and treatment program; (b) promoting client understanding of the goals of the early and periodic screening diagnosis and treatment program and the services this program provides; (c) providing a better understanding among eligible households of the importance of preventive health care; (d) informing eligible households of the supportive services available from local social services officials, and from public and private organizations, to enable such households to participate in the early and periodic screening diagnosis and treatment program, and how eligible households may obtain these supportive services; (e) providing eligible households with a list of providers participating in the early and periodic screening diagnosis and treatment program; and (f) assisting eligible households in recognizing the importance of a continuing relationship with appropriate medical providers and assisting such households in establishing this relationship. 2. In order to accomplish the objectives set forth in subdivision one of this section, early and periodic screening diagnosis and treatment outreach demonstration project proposals shall include: (a) plans for achieving personal contact with eligible households; (b) the number of staff positions to be included in such an outreach demonstration project, and the job descriptions of these staff positions; (c) the follow up services to be provided to help assure that appointments are kept; (d) health education and informational materials to be provided to eligible households and the methods to be utilized in making such materials available to eligible households; (e) plans for media announcements; (f) the extent of the involvement, if any, of participating households in promoting the goals of the early and periodic screening diagnosis and treatment program; (g) methods for reaching illiterate, blind or other parents for whom written material is inappropriate; and (h) any other information the department may deem appropriate. 3. The department is authorized to provide technical assistance to public and/or private organizations submitting plans for and/or conducting early and periodic screening diagnosis and treatment outreach demonstration projects, in order to help assure effective implementation of such projects. 4. The department is authorized to promulgate rules and regulations and to take any other measures it deems necessary for the effective implementation of the early and periodic screening diagnosis and treatment outreach demonstration projects.5. Non federal expenditures for the purposes specified in this section shall be fully borne by the state. * NB Expired June 30, 1983
