§ 15C-5. Change of name, address, or telephone number.

§ 15C‑5.  Change ofname, address, or telephone number.

(a)        A programparticipant shall notify the Attorney General within 30 days after the programparticipant has obtained a legal name change by providing the Attorney Generala certified copy of any judgment or order evidencing the change or any otherdocumentation the Attorney General deems to be sufficient evidence of the namechange. If the program participant fails to notify the Attorney General of aname change in the manner provided in this subsection, the Attorney Generalshall cancel the certification of the program participant in the Program.

(b)        A programparticipant shall notify the Attorney General of a change in address ortelephone number from the address or telephone number listed for the programparticipant on the application at least seven days before the change occurs. Ifthe program participant fails to notify the Attorney General of a change inaddress or telephone number in the manner provided in this subsection, theAttorney General shall cancel the certification of the program participant inthe Program.
