§58‑50‑35.  Notice of nonpayment of premium required beforeforfeiture.

No insurance company doingbusiness in this State and issuing health and/or accident insurance policies,other than contracts of group insurance or disability and/or accidental death benefitsin connection with policies of life insurance, the premium for which is to becollected in weekly, monthly, or other periodical installments by authority ofa payroll deduction order executed by the assured and delivered to suchinsurance company or the assured's employer authorizing the deduction of suchpremium installments from the assured's salary or wages, shall, during theperiod for which such policy is issued, declare forfeited or lapsed any suchpolicy hereafter issued or renewed until and unless a written or printed noticeof the failure of the employer to remit said premium or installment thereofstating the amount or portion thereof due on such policy and to whom it must bepaid, has been duly addressed and mailed to the person who is insured undersuch policy at least 15 days before said policy is canceled or lapsed.
