5123.0415 Licensees to notify director of change of address.

As used in this section, “license” means a license, certificate, or evidence of registration.

Each person and government entity that applies for or holds a valid license issued under section 5123.082, 5123.161, 5123.19, 5123.45, 5126.25, or 5126.252 of the Revised Code shall notify the director of developmental disabilities of any change in the person or government entity’s address.
