SECTION 23-17.4-15.7

   § 23-17.4-15.7  Service plans. – (a) Within a reasonable time after move-in, the administrator shall beresponsible for the development of a written service plan based on the initialassessment. The service plan shall include at least:

   (1) The services and interventions needed;

   (2) Description, frequency and duration of the service orintervention; and

   (3) Party responsible for arranging the service.

   (b) The service plan shall be developed by a qualified personand shall be signed by both parties. The service plan shall be reviewed by bothparties periodically and each time a resident's condition changes significantlyand all changes shall be acknowledged in writing by both parties.
