   § 23-19.12-9  Application for permit.– Application for a permit shall be made to the director of the department ofenvironmental management upon a form prescribed by the director. Upon approvalof the application by the director, a permit shall be issued. Permits shallexpire three (3) years from the date of issue, unless sooner if suspended orrevoked. The director of the department of environmental management isempowered to charge an annual application fee for a medical waste transporterspermit.
