SECTION 31-26-10

   § 31-26-10  Accident report forms –Contents. – The division of motor vehicles shall prepare and upon request supply to policedepartments and other suitable agencies or individuals, forms for accidentreports required by this chapter, appropriate with respect to the personsrequired to make those reports and the purposes to be served. The writtenreports to be made by persons involved in accidents and by investigatingofficers shall call for sufficiently detailed information to disclose withreference to a traffic accident the cause, conditions then existing, and thepersons and vehicle involved.
