§ 39-11-5 - Application for certificate – Fee.
SECTION 39-11-5
§ 39-11-5 Application for certificate– Fee. – Application for a certificate shall be in writing and shall contain suchinformation as the administrator may require. All applications for acertificate shall be in the form prescribed by the administrator and shall beaccompanied by a filing fee of twenty dollars ($20.00).
