SECTION 40.1-24-4

   § 40.1-24-4  Application for license.– An application for a license shall be made to the department upon formsprovided by it and shall contain such information as the department reasonablyrequires, which may include affirmative evidence of ability to comply with suchreasonable standards, rules, and regulations as are lawfully prescribedpursuant to this chapter.
