   § 5-59.1-5  Application for orthotic orprosthetic license. – Any person who desires to be licensed as set forth in § 5-59.1-4 shall inwriting submit an application on forms provided by the department for a licenseaccompanied by a fee of three hundred and thirty dollars ($330) with all othercredentials that the department requires and as required by this chapter. Allthe proceeds of any fees collected pursuant to the provisions of this chaptershall be deposited as general revenues.
