SECTION 7-6-101

   § 7-6-101  Forms to be furnished bysecretary of state. – All reports required by this chapter to be filed in the office of the secretaryof state shall be made on forms which shall be prescribed and furnished by thesecretary of state. Forms for all other documents to be filed in the office ofthe secretary of state shall be furnished by the secretary of state on requestfor them, but their use, unless otherwise specifically prescribed in thischapter, is not mandatory.
