§ 362. Application; undertaking; recommendation

Application for such permit shall be made in writing, signed by the applicant, to the liquor control board on a form prescribed by the board, containing the name, residence and business address of the applicant, the name and address of the vendor to be represented by the applicant, and an undertaking by the applicant to comply with the regulations of the board. Such application shall have appended thereto a recommendation of the applicant as being qualified to hold such permit, signed by such vendor.
