§ 32.1-129. Application for license.

Each application for a hospital or nursing home license shall be made on aform prescribed by the Board. The application shall specify the official nameand the kind of hospital or nursing home, the location thereof, the name ofthe person in charge and such additional relevant information as the Boardrequires.
