40.1-96 - Contents of employment certificates.

§ 40.1-96. Contents of employment certificates.

The employment certificate required to be issued shall state the name, sex,date of birth and place of residence of the child. It shall certify that allthe conditions and requirements for issuing an employment certificate underthe provisions of this chapter have been fulfilled and shall be signed by theperson issuing it. It shall state the kind of evidence of age accepted forthe employment certificate. Except for work coming within one of theexceptions in § 40.1-79.01, the certificate shall show the name and addressof the employer for whom and the nature of the specific occupation in whichthe employment certificate authorizes the child to be employed and shall bevalid only for the occupation so designated. It shall bear a number, shallshow the date of its issue, and shall be signed by the child for whom it isissued in the presence of the person issuing it. It shall be issued intriplicate, one copy to be mailed to the employer, one copy to be sent to theCommissioner and one copy to be retained and kept on file by the issuingofficer.
