§ 46.2-374. Department to prepare and supply forms for reports.

The Department shall prepare and, on request, supply to police departments,medical examiners or other officials exercising like functions, sheriffs, andother suitable agencies forms for accident reports and other reports requiredto be made to the Department, appropriate with respect to the personsrequired to make the reports and the purpose to be served. The forms foraccident reports shall include suitable spaces for the name or names of theinsurance carrier of the automobile liability policy of each vehicle involvedin the accidents as required to be reported by § 46.2-373.
