      Sec. 38a-574. Standard underwriting form. (a) On or before July 1, 1993, the board of directors of the Connecticut Small Employer Health Reinsurance Pool shall establish, subject to the approval of the Insurance Commissioner, a standard underwriting form for use by small employer carriers for medical underwriting of health insurance plans and insurance arrangements covering small employers, as defined in section 38a-564. Within ninety days after approval by the Insurance Commissioner of the standard underwriting form the board shall require every small employer carrier, as a condition of transacting such business in this state, to use the form for medical underwriting of such plans and arrangements.

      (b) The form may be amended from time to time as the board deems necessary, subject to the approval of the Insurance Commissioner.
